Care Concierge at Carrum Health

The heart of Carrum's solution is a seamless and highly personalized patient experience enabled by our “Care Concierge” who provides comprehensive non-medical support throughout the entire episode of care. The Care Concierge is the patient’s biggest advocate and creates a welcoming and comforting environment from establishing the initial relationship with the patient to preparing them for surgery to supporting the recovery process. The intent is to deliver a drastically improved experience compared to navigating the healthcare system alone.

• Serve as the single face and voice of the entire Carrum Health experience for the member throughout their journey. You’ll pull out all the stops to deliver a memorable experience (in the “I can’t wait to tell my friends about this fabulous service” kind of way).
• Offer instruction, support and guidance in choosing a provider and preparing the patient for all clinical appointments. You’ll be navigating patients and their families through the complexities of healthcare at a time when they need help the most.
• Facilitate medical records gathering, scheduling and travel to ensure that logistics are seamlessly coordinated and communicated to the member.
• Manage post-discharge care planning with physicians and transition of the patient between various clinical settings to ensure a smooth recovery and return to health.

• Excellent interpersonal skills: courteous, friendly, helpful, empathetic, a good listener and service oriented
• Exceptional written and oral communication skills
• High degree of professionalism including the ability to keep a cool head and discretion in working with confidential information
• Superior judgment, problem solving and organizational skills
• Solid computer skills including a strong understanding of Microsoft Office applications (e.g., Word, Excel, PowerPoint), email and internet browsers
• Nursing (RN) or other clinical background preferred but not required
